Core Requirements

Declare the Incident

Assign an incident owner, severity, start time, affected services, affected Providers, and immediate operational risk.

Stabilize Operations

Protect care delivery, stop harmful processing, preserve data, and communicate approved workarounds.

Preserve Evidence

Retain logs, request IDs, transaction IDs, screenshots, timestamps, deployments, and relevant audit history.

Coordinate Response

Engage technical, Provider operations, privacy, security, billing, payroll, or legal stakeholders as needed.

Close and Improve

Record root cause, impact, remediation, validation, lessons learned, and preventive actions.
