Incidents & Reports

Purpose

Incidents & Reports supports medication-related incident documentation, immediate action, notification, follow-up, and monthly reporting.

HIPAA, Clinical Governance, and Proprietary Training Notice

Medication information contains highly sensitive protected health information and must be handled under the Provider's HIPAA policies, role-based access rules, minimum necessary standard, and applicable clinical requirements.

To protect medication safety procedures and the proprietary workflow of True Care System, selected advanced functions are intentionally not published in full. Official Provider customers receive direct, role-based training from the True Care System team for licensed nurses and other personnel who are formally authorized by the Provider to manage or administer medications.

Incident Response Framework

01Protect the Individual

Take the immediate action required by the clinical situation and Provider policy.

02Document the Event

Record objective facts, medication, severity, description, and action taken.

03Notify Required Parties

Document authorized clinical, supervisory, physician, family, or regulatory notification.

04Complete Follow-up

Record monitoring, corrective action, investigation, and closure when required.

Severity Review

Low

Requires documentation and routine follow-up under Provider policy.

Moderate

May require clinical review, notification, monitoring, or corrective action.

High / Critical

Requires immediate response and escalation under applicable policy and requirements.

Restricted Investigation Procedures

The complete workflow for clinical escalation, formal investigation, corrective action, physician or family notification, quality review, regulatory reporting, and incident closure is reserved for direct Provider training.

Direct Training Scope

Official Provider customers receive direct incident-management training covering classification, severity, immediate action, notification, investigation, follow-up, reporting, audit documentation, and HIPAA safeguards.
