MEDICATION MANAGEMENT

Medication

Purpose

The Medication module supports physician orders, Medication Administration Records, treatment documentation, PRN administration, vital signs, inventory tracking, controlled-substance oversight, and medication-related incident reporting.

HIPAA, Clinical Safety, and Direct Provider Training Notice

Medication management is one of the most sensitive clinical and HIPAA-related areas within True Care System. To protect Individual health information, medication safety, Provider-specific clinical procedures, and proprietary compliance workflows, some advanced functions are intentionally not described in full within this public guide.

When a Provider becomes an official True Care System customer, the True Care System team will provide direct, role-based training to the Provider's licensed nurses and other personnel formally authorized and qualified to administer or manage medications under the Provider's policies and applicable requirements.

Clinical Responsibility

True Care System supports documentation and operational workflows. Medication orders, administration, assessment, delegation, monitoring, and follow-up must be completed only by personnel authorized under the Provider's policies, professional scope, physician orders, and applicable laws and regulations.

The Medication menu contains six primary workspaces. These pages follow the visible menu structure currently available in True Care System.

Orders

Orders is the master medication profile for an Individual. It displays medication, route, order type, schedule, instructions, start and end dates, status, prescriber, pharmacy, dose information, and related clinical notes.

Add Order

The Add Order form supports medication identity, form, strength, route, dose, schedule, frequency, status, administration times, PRN reason, directions, special instructions, prescription information, prescriber, pharmacy, and clinical notes.

Detailed order validation, clinical review, authorization, correction, discontinuation, and medication-change procedures are included in direct Provider training.

MAR

The Medication Administration Record provides a monthly view of scheduled medication doses and saved administration outcomes for the selected Individual.

The complete administration decision process, late or missed dose response, medication refusal handling, error escalation, correction procedures, and clinical follow-up are reserved for authorized Provider training.

Treatment Record

Treatment Record supports monthly treatment tracking and a Nursing Monthly Summary. The screen includes service or treatment categories, daily treatment cells, Individual demographic information, diagnosis, allergies, plan-of-care review, nursing comments, and signature information.

PRN & Vitals

PRN & Vitals supports documentation of as-needed medication administration, dose, route, reason, result or response, notes, and related vital signs.

PRN assessment, effectiveness review, repeat-dose restrictions, notification thresholds, vital-sign interpretation, and escalation procedures are covered through direct clinical training.

Inventory & Controlled

Inventory & Controlled tracks the current quantity on hand and supports medication transactions such as receive, dispense, waste, and adjust.

Controlled-substance counting, witnessing, discrepancy response, waste procedures, reconciliation, investigation, and audit requirements are not published in full and require direct Provider training.

Incidents & Reports supports medication-related incident documentation, severity, medication name, description, immediate action, reported-by information, notification, follow-up, and monthly reporting.

Formal investigation, clinical escalation, physician or family notification, corrective action, quality review, and regulatory reporting procedures are included in direct Provider training and must follow the Provider's approved policies.

General Medication Workflow

  1. Select the correct Individual.
  2. Review active physician medication orders.
  3. Confirm medication identity, route, dose, schedule, and order status.
  4. Review MAR or treatment documentation for the selected month.
  5. Record medication administration only when authorized and appropriate.
  6. Record PRN reason and response when applicable.
  7. Update medication inventory through the appropriate transaction type.
  8. Document medication incidents and immediate actions when required.
  9. Complete follow-up, notification, and reporting under Provider policy.

Validation Checklist

  • The correct Provider and Individual are selected.
  • The medication matches the active physician order.
  • The medication name, strength, form, route, and dose are correct.
  • The scheduled or PRN order type is correct.
  • Administration time and status are accurate.
  • PRN reason and response are complete when required.
  • Inventory quantity and transaction type are accurate.
  • Medication incidents are documented promptly and objectively.
  • Required notifications and follow-up are completed.
  • Only authorized staff perform medication-management actions.

Security and HIPAA

Medication records contain highly sensitive protected health information and require strict role-based access, minimum necessary use, and auditability.

  • Access medication records only for an approved care or operational purpose.
  • Do not share medication screenshots through unapproved channels.
  • Verify the Individual before reviewing or entering medication information.
  • Do not document information unrelated to the medication or clinical event.
  • Protect prescriber, pharmacy, diagnosis, allergy, and treatment information.
  • Record access, changes, deletions, and administrative actions through Audit Logs where applicable.
  • Follow the Provider's medication-administration and incident-reporting policies.
