Release notes may reference Provider operations, PHI-related workflows, security controls, billing, payroll, EVV, integrations, or proprietary implementation details. Public documentation must avoid credentials, private endpoints, source code, customer data, and sensitive internal security information.
Purpose
Provides a controlled list of confirmed limitations, temporary workarounds, affected versions, and planned resolution status.
Release Documentation Requirements
Issue Identification
Record a unique issue ID, affected module, version, environment, Provider impact, and severity.
Symptoms
Describe observable behavior without exposing sensitive internal data.
Workaround
Provide only an approved, safe, and reversible workaround.
Risk
State whether the issue affects PHI, billing, payroll, EVV, Provider isolation, data integrity, or availability.
Status
Use Open, Investigating, Fix in Progress, Ready for QA, Scheduled, Resolved, or Closed.
